Visit Requirements

Various insurance companies require an inter-office referral which you must bring with you should this not have been received (by the office) in the days preceding the appointment. If a co-payment is due for your visit, the co-payment is due the day of your appointment.

If you have had any testing (MRI, X-Rays, CT scan and/or an EMG) done, please bring a copy of the report along with the actual study if applicable. You may pick up these films at the imaging center where the test was performed. Please call the facility 24 hours in advance to have your films or compact discs ready.

Payments & Billing

Thank you for choosing us as your healthcare provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered part of your treatment.

We accept cash, checks, Visa/Mastercard.

A NSF fee of $25.00 will be charged for all insufficient funds.

All co-pays and deductibles are due at the time of service. Please notify our staff of any insurance changes prior to your appointment.
